Job Description

Case Worker, Occupational Therapist, Westminster  
 
Local authority experience. Knowledge of equipment, Major and minor adaptations. Experience of functional assessment experience of working with people with disabilities/vulnerable people. Manage a caseload of complex cases, including clients with multiple physical, cognitive, and environmental challenges requiring advanced OT input. Conduct in-depth home visits to assess functional needs, environmental risks, and suitability for adaptations or equipment. Complete manual handling assessments, including risk assessments and safe handling plans involving hoists, slings, and transfer aids. Prescribe and oversee major adaptations (e.g. level access showers, stairlifts, ramps, through-floor lifts) in collaboration with housing and DFG teams. Provide clinical reasoning and justification for high-cost equipment and structural changes, ensuring compliance with legislation and best practice. Liaise with contractors, surveyors, and housing officers to ensure adaptations meet the client’s needs and are delivered safely and effectively. Support discharge planning and hospital avoidance by ensuring timely provision of essential equipment and adaptations. Maintain accurate documentation and contribute to legal reports, safeguarding enquiries, and funding panels where required. Deliver supervision and peer support to junior staff, promoting safe manual handling practices and reflective learning. To deliver the best service for our residents, this role is office based, working along side the Team Manager. Visits to residents homes will be required.  
 
£32.50 per hour
