Adult Social Worker, CHC Team, Cheshire West 
Experienced Adult Social Worker needed for the Continuing Health Care team. 
Experience in CHC required. 
Candidates need to be available for up to 4 days a week in the locality for face to face work. 
£ 35 per hour.
